As part of the interfederal integrated care programme, we are looking for a passionate midwife to carry out the mission of Care and Assistance Coordinator. This programme, called 'Born in Belgium', aims to guarantee effective access to care for pregnant women and their children facing psychosocial vulnerabilities.
What does the job consist of?
As a Born in Belgium midwife, you become the common thread of the perinatal pathway. This function is based on several key axes to enrich your practice: it will allow you to combine your clinical expertise with a central role of coordination, in the service of better continuity of care and reinforced support for families. As a true conductor, you connect the actors and give meaning to each patient's journey, in particular through the following dimensions:
- Identification and expertise in screening
You identify vulnerabilities (mental health, housing, financial situation, etc.) early on thanks to the innovative Born in Belgium Pro tool, in order to anticipate and adapt the support from the first contacts.
- Structuring and mobilization of the network
You will build and activate a tailor-made multidisciplinary team around each patient: midwives, gynaecologists, psychologists, social services, ONE and other partners as needed.
- Care plan coordination
You will develop a personalised support plan and ensure its concrete implementation, guaranteeing coherence, fluidity and continuity of interventions.
- Facilitation and leadership
You will organise and lead perinatal consultations (face-to-face or remote), promoting collaboration and collective decision-making.
- Continuity and trust
You will support families over the long term, ensuring a smooth transition between prenatal, birth and early childhood, as a resource person and reference person.

Training and prerequisites
- Recognized midwifery diploma with visa and INAMI number
- A first experience in coordination is an asset
Experience and knowledge
- Experience in a mother-child department (maternity, delivery room, pediatrics) is a plus
- Good command of computer tools (MS Office)
- Knowledge of the second national language and English
- Ease with communication tools and media
