
Health Advisor
Job description
As a Health Relay Advisor, you will ensure the care of a more vulnerable population by guiding and advising them in relation to their psycho-medico-social problems and ensure follow-up with Partenamut's internal services (social, ergo, indem, insurance, etc.) and with external partners (VAD, CPAS, care providers, etc.). You will play a central role as a mediator between health issues, life courses and existing resources.
👉 On a daily basis:
You guide and support clients in vulnerable situations, with a particular focus on mental health, helping them to put down their emotional charge and identify concrete courses of action.
You are one of the faces of our Health Relays : thematic permanences, listening points, referrals to internal services (social, insurance, allowances, etc.) and external partners (CPAS, home care, non-profit organisations, care providers, etc.).
You co-construct and bring these relays to life with agility: you test, adjust, propose. You participate in meetings/actions/working groups by being a force for proposing new ideas for partnerships and services, and support for these populations.
You will participate in the creation of content and actions related to health pathways (education, prevention, autonomy).
You coordinate events, workshops, and speakers (planning, logistics, communication). In this coordination role, you will also be required to manage the organization of workshops, event schedules and partner permanences.
You are also a detector of potential partnerships , in connection with the themes you support.
Profile
⭐ Your profile as a Health Relay Advisor:
You have expertise in mental health: you know how to listen with empathy, welcome emotions and guide in a caring and structured way. You know the issues and are familiar with the main pathologies related to the theme and you can make use of this experience (mandatory). 💙
You can have a minimum of 3 years of paramedical experience (mandatory)
You have your B licence. Mobility is essential for this function. 🚗
You have a perfect command of French, both orally and in writing.
You are comfortable with digital tools, including Odoo (or motivated to learn it). 💻
You are a good active listener and customer-oriented.
You are able to manage the emotions of the people in front of you and you easily put yourself in the role of advisor.
You can work both in a team and individually, you are change-oriented and have a strong sense of respect and quality.
You have a knowledge of Project Management and a strong ability to adapt.
