Job description

  • You will ensure the care of a more vulnerable population by guiding and advising them in relation to their psycho-medico-social problems and ensure follow-up with Partenamut's internal services (social, ergo, indem, insurance, etc.) and with external partners (VAD, CPAS, etc.);

  • You will provide guidance to people according to the themes of health pathways in the form of permanence in the Health Relays (oncology, Alzheimer's disease, cardiology, mental health );

  • You will actively participate in the creation and implementation of health-related actions within Partenamut (health pathways, specific permanences, health literacy content, event, care);

  • You participate in meetings/ actions/working groups by proposing new ideas, partnerships and services, and support for these populations;

  • You will also ensure the coordination of the prevention area with the meeting and recruitment of non-profit organisations or health partners;

  • In this coordination role, you will also be required to manage the organization of workshops, event schedules and partner offices;

  • Finally, you may be required to ensure the construction of education, prevention and autonomy components (around predefined health themes);

  • In the event of a beosin, you strengthen the ERGO and 1st line teams in their activities;


  • You are trained as an Occupational Therapist and/or have at least 3 years of professional experience in a paramedical function;

  • You have your driver's license and don't have a problem getting around as part of your job;

  • You are fluent in French, both oral and written;

  • You are comfortable with different IT tools and are able to work with Odoo (or you are willing to learn it);

  • You are a good active listener and customer-oriented;

  • You are able to manage the emotions of the people in front of you and you easily put yourself in the role of advisor;

  • You can work both as part of a team and individually, you are change-oriented and you have a strong sense of respect and quality;

  • You have some knowledge of Project Management and a strong ability to adapt;
