Under a full-time fixed-term contract (38 hours/week)

What does the job consist of?

As a social worker, your mission will be to assist patients on a psychosocial, educational and administrative level in order to improve their conditions of stay and to ensure their discharge from the institution in optimal conditions.

You will work in close collaboration with the entire multidisciplinary team of the unit (medical, caregiving, paramedical team, etc.) in order to ensure, together, the improvement of the patient's overall state of health.

More precisely;

·      You provide assistance to patients and their families:

o Administrative by establishing patients' social files (gathering patient data, keeping an up-to-date report of each patient in the unit, filling in various other forms for the patient, etc.)

o By ensuring contacts with external services (CPAS, mutual insurance companies, etc.)

o By arranging transfers, returns or home help/services for patients

o By promoting liaison between the patient, his family/entourage and the medical, paramedical and nursing profession, both within and outside the hospital

·      You provide psychosocial support by teaching patients to self-care in the context of their pathology and treatment

Your manager, your team and your environment

·      You will join a team of 5 social workers and report to the head of the social service, Mrs. Bertinchamps Virginie.

·      You will work in the different units of the Joseph Bracops Hospital located in Anderlecht.

·      The team will offer you the sharing, listening, and support you need so that you can quickly feel comfortable in the function and within our hospital. A personalized integration process will be carefully carried out by Virginie.

Your education

·      You are in possession of a Bachelor's degree in social work (graduate).

Your experience and knowledge

·      You are looking for a first or new experience.

·      Experience in a hospital or care sector setting is an asset.

·      You have a good knowledge of the Brussels social network (external bodies such as CPAS, mutual insurance companies, insurance companies, etc.), social legislation and social measures specific to health.

·      You have a good knowledge of Dutch: You are able to understand the patient's needs and communicate with your colleagues.

·      Your computer knowledge allows you to ensure optimal follow-up of files.

·      Listening and human support : you create a relationship of trust with patients and their loved ones thanks to your empathy, active listening and appropriate communication. You promote the autonomy of the people you support.

·      Analysis and solution orientation : you are able to assess a social situation, identify priorities and propose appropriate solutions by mobilizing available resources.

·      Organization and rigor : you ensure a precise follow-up of files, manage your priorities efficiently and guarantee the quality of the information transmitted.

·      Collaboration and networking : You enjoy multidisciplinary work, share your expertise and develop effective collaborations with internal and external partners.

·      Ethics and professionalism : you demonstrate discretion, integrity and respect in the management of sensitive situations, placing the patient at the centre of your actions.

We offer you:

·      Type of contract: 6-month fixed-term contract

·      Working time: 100% (38h/week)

·      Opening hours: Monday to Friday from 8 a.m. to 4 p.m.

Remuneration on the basis of the IFIC scales (category 14), with resumption of seniority:

·      Min: 3379.86€ (index: 2.1647)

·      Max: 5339.19€ (index 2.1647)
